ABSTRACT

Social distancing measures and shelter-in-place orders to limit mobility and transportation were among the strategic measures taken to control the rapid spreading of COVID-19. In major metropolitan areas, there was an estimated decrease of 50 to 90 percent in transit use. The secondary effect of the COVID-19 lockdown was expected to improve air quality, leading to a decrease in respiratory diseases. The present study examines the impact of mobility on air quality during the COVID-19 lockdown in the state of Mississippi (MS), USA. The study region is selected because of its non-metropolitan and non-industrial settings. Concentrations of air pollutants-particulate matter 2.5 (PM2.5), particulate matter 10 (PM10), ozone (O3), nitrogen oxide (NO2), sulfur dioxide (SO2), and carbon monoxide (CO)-were collected from the Environmental Protection Agency, USA from 2011 to 2020. Because of limitations in the data availability, the air quality data of Jackson, MS were assumed to be representative of the entire region of the state. Weather data (temperature, humidity, pressure, precipitation, wind speed, and wind direction) were collected from the National Oceanic and Atmospheric Administration, USA. Traffic-related data (transit) were taken from Google for the year 2020. The statistical and machine learning tools of R Studio were used on the data to study the changes in air quality, if any, during the lockdown period. Weather-normalized machine learning modeling simulating business-as-scenario (BAU) predicted a significant difference in the means of the observed and predicted values for NO2, O3, and CO (p < 0.05). Due to the lockdown, the mean concentrations decreased for NO2 and CO by -4.1 ppb and -0.088 ppm, respectively, while it increased for O3 by 0.002 ppm. The observed and predicted air quality results agree with the observed decrease in transit by -50.5% as a percentage change of the baseline, and the observed decrease in the prevalence rate of asthma in MS during the lockdown. This study demonstrates the validity and use of simple, easy, and versatile analytical tools to assist policymakers with estimating changes in air quality in situations of a pandemic or natural hazards, and to take measures for mitigating if the deterioration of air quality is detected.

ABSTRACT

During the COVID-19 pandemic in Mississippi in 2020 and 2021, nonurgent elective procedures requiring hospitalization were halted three times to preserve the state's hospital resources. To evaluate the change in hospital intensive care unit (ICU) capacity after the implementation of this policy, we analyzed Mississippi's hospital discharge data. We compared daily mean ICU admissions and census for nonurgent elective procedures between three intervention periods and baseline periods corresponding to Mississippi State Department of Health executive orders. We further evaluated the observed and predicted trends, using interrupted time series analyses. Overall, the executive orders reduced the mean number of ICU admissions for elective procedures from 13.4 patients to 9.8 patients daily (a 26.9 percent decline). This policy also decreased the mean ICU census for nonurgent elective procedures from 68.0 patients to 56.6 patients daily (a 16.8 percent decline). The state managed to free, on average, eleven ICU beds daily. Postponing nonurgent elective procedures in Mississippi was a successful strategy that resulted in a decline in ICU bed use for nonurgent elective surgeries during times of unprecedented stress on the health care system.

ABSTRACT

Background and Objectives: Guidance around maternity care practices and infant feeding during the COVID-19 pandemic changed over time and was sometimes conflicting. Hospital maternity practices influence breastfeeding, an important preventive strategy against viral illness. Most birthing hospitals in Mississippi are enrolled in CHAMPS, a quality improvement initiative to support breastfeeding and continuously collect maternity care data. The aims of this study were to (1) assess changes to maternity care policies in response to COVID-19, and (2) compare hospital-level breastfeeding, skin-to-skin, and rooming-in rates, at cohort hospitals, before and during the pandemic, overall and stratified by race. Methods: Hospitals responded to a survey on maternity policies in May and September 2020 (Aim 1); hospitals submitted data on breastfeeding and maternity care practices before and during the pandemic (Aim 2). We tested for differences in survey responses using chi-squared statistics and performed an interrupted time series analysis on breastfeeding and maternity care practices data. Results: Twenty-six hospitals responded to the May and September 2020 surveys. Hospitals used different sources to create maternity care policies, and policies differed between institutions. Trends in rates of any and exclusive breastfeeding in the hospital cohort plateaued during the pandemic, in comparison to previous gains, and rates of skin-to-skin and hospital rooming-in decreased. No differences were evident between races. Conclusions: Policies (Aim 1) and practices in the quality improvement cohort hospitals were inconsistent during the COVID-19 pandemic, and changes measured to practices were detrimental (Aim 2). Ongoing monitoring is recommended.

ABSTRACT

The COVID-19 pandemic has created a severe upheaval in the U.S., with a particular burden on the state of Mississippi, which already has an exhausted healthcare burden. The main objectives of this study are: (1) to analyze the county-level COVID-19 cases, deaths, and vaccine distribution and (2) to determine the correlation between various social determinants of health (SDOH) and COVID-19 vaccination coverage. We analyzed COVID-19-associated data and county-level SDOH factors in 82 counties of Mississippi. The cumulative COVID-19 and socio-demographic data variables were grouped into feature and target variables. The statistical and exploratory data analysis (EDA) was conducted using Python 3.8.5. The correlation between the target and feature variables was performed by Pearson Correlation analysis. The heat Map Correlation Matrix was visually presented to illustrate the correlation between each pair of features and each target variable. Results indicated that people of Asian descent had the highest vaccination coverage of 77% fully vaccinated compared to 52%, 46%, 42% and 25% for African Americans, Whites, Hispanics, and American Indians/Alaska Natives, respectively. The county-level vaccination rate was significantly higher among the minority populations than the White population. It was observed that COVID-19 cases and deaths were positively correlated with per capita income and negatively correlated with the percentage of persons without a high school diploma (age 25+). This study strongly demonstrates that different SDOH factors influence the outcome of the COVID-19 vaccination rate, which also affects the total number of COVID-19 cases and deaths. Vaccine promotion should be given to all populations regardless of race and ethnicity to achieve uniform acceptance. Therefore, statewide policy recommendations focusing on specific community needs should help achieve health equity in COVID-19 vaccination management.

ABSTRACT

We aimed to study the disparity in the clinical profile and outcomes of hospitalized Multisystem Inflammatory Syndrome in Children (MIS-C) patients at our center. The second goal was to examine the temporal association with preceding SARS-CoV-2 infection by race/ethnicity in our community in Mississippi. We found the racial disparity in the prevalence of MIS-C exceeded its temporal association with SARS-CoV-2 infections. We included 51 consecutive MIS-C patients hospitalized, whose median age was 9 (interquartile range [IQR] 5-12) years, 58% were male, 71% were black, 25% were white, and 4% belonged to other groups. We found a delay between onset of symptoms and hospitalization in black patients compared with white patients with a median of 2 (IQR 0-7) vs median of 0 (0-5) urgent care visits (P = .022), respectively. Black patients were hospitalized longer (median 8, IQR 2-39 days) than whites (median 5, IQR 3-14 days), P = .047. A total of 38.9% of blacks and 23.1% of whites were admitted to intensive care unit (P = .498); 36.1% of blacks had severe cardiac involvement vs 23.1% of white patients, P = .531. Future studies of MIS-C are required to improve health equity for children.

BACKGROUND: Racial disparities in SARS-CoV-2 infection, hospitalization, and multisystem inflammatory syndrome in children (MIS-C) have been reported. However, these reports have been based on incomplete data relying on passive reporting, unknown catchment populations, and unknown infection prevalence. We aimed to characterize population-based incidence of MIS-C and COVID-19 hospitalizations among non-Hispanic Black and White children using active surveillance based on seroprevalence-based cumulative incidence of pediatric SARS-CoV-2 infection in a defined catchment 16-county area of Mississippi. METHODS: Active, population-based surveillance for MIS-C and acute COVID-19 hospitalizations meeting clinical and laboratory criteria was conducted by adjudicating clinicians at the major pediatric referral hospital for Mississippi, University of Mississippi Medical Center, from March 2020, to February 2021. Race-stratified SARS-CoV-2 seroprevalence was estimated using convenience samples from persons <18 years to calculate cumulative SARS-CoV-2 infections in the population. RESULTS: Thirty-eight MIS-C cases and 74 pediatric acute COVID-19 hospitalizations were identified. Cumulative incidence of MIS-C was 4.7 times higher among Black compared with White children (40.7 versus 8.3 cases per 100,000 SARS-CoV-2 infections). Cumulative incidence of COVID-19 hospitalization was 62.3 among Black and 33.1 among White children per 100,000 SARS-CoV-2 infections. CONCLUSIONS: From the same catchment area, active surveillance, and cumulative incidence of infection estimated by seroprevalence, we show strikingly higher incidence of SARS-CoV-2-hospitalization and MIS-C in non-Hispanic Black children compared with White children before COVID-19 vaccination introduction in children. These disparities in SARS-CoV-2 manifestations cannot be accounted for by differences in exposure or testing. Targeted vaccine interventions will lessen disparities observed with SARS-CoV-2 manifestations in children.

OBJECTIVE: To evaluate differences between survivors versus nonsurvivors undergoing mechanical ventilation for coronavirus disease 2019 (COVID-19)-associated respiratory failure at two community medical centers. METHODS: This was a multicenter, retrospective cohort analysis of all adult patients mechanically ventilated for COVID-19-associated respiratory failure in two community hospital intensive care units in southern Mississippi from March 15, 2020 through October 10, 2020. RESULTS: Among 56 patients requiring mechanical ventilation, the mortality rate was 75% (42/56). Expired patients were intubated later (2 vs 5 days, 95% confidence interval [CI] 6.314-0.8041, P = 0.0983), had lower PaO2:FiO2 ratios (65 vs 77.5 mm Hg, 95% CI 36.08-59.03, P = 0.6305), and tolerated lower levels of positive end-expiratory pressure (7.9 vs 12.6 cm H2O, 95% CI 0.1373-6.722, P = 0.0415) at the time of intubation. CONCLUSIONS: Our results suggest that earlier intubation may be associated with reduced mortality in patients with COVID-19-associated respiratory failure and should be further evaluated in the form of a randomized controlled trial.

ABSTRACT

PURPOSE: To quantify and compare SARS-CoV-2 transmission potential across Alabama, Louisiana, and Mississippi and selected counties. METHODS: To determine the time-varying reproduction number Rt of SARS-CoV-2, we applied the R package EpiEstim to the time series of daily incidence of confirmed cases (mid-March 2020 - May 17, 2021) shifted backward by 9 days. Median Rt percentage change when policies changed was determined. Linear regression was performed between log10-transformed cumulative incidence and log10-transformed population size at four time points. RESULTS: Stay-at-home orders, face mask mandates, and vaccinations were associated with the most significant reductions in SARS-CoV-2 transmission in the three southern states. Rt across the three states decreased significantly by ≥20% following stay-at-home orders. We observed varying degrees of reductions in Rt across states following other policies. Rural Alabama counties experienced higher per capita cumulative cases relative to urban ones as of June 17 and October 17, 2020. Meanwhile, Louisiana and Mississippi saw the disproportionate impact of SARS-CoV-2 in rural counties compared to urban ones throughout the study period. CONCLUSION: State and county policies had an impact on local pandemic trajectories. The rural-urban disparities in case burden call for evidence-based approaches in tailoring health promotion interventions and vaccination campaigns to rural residents.

CDC recommends COVID-19 vaccination for women who are pregnant, recently pregnant, trying to become pregnant, or might become pregnant in the future.*,† This report describes 15 COVID-19-associated deaths after infection with SARS-CoV-2 (the virus that causes COVID-19) during pregnancy in Mississippi during March 1, 2020-October 6, 2021.

ABSTRACT

The COVID-19 pandemic impacted the health care system in unprecedented ways. We reviewed the registry of new cancer patients who presented to the Children's of Mississippi Center for Cancer and Blood Disorders and showed the average number of new pediatric cancer diagnoses dropped during the initial COVID-19 months and rose significantly in June 2020. We must encourage families to seek health care when needed and keep scheduled appointments for routine vaccinations and health maintenance as we know the long-term sequela of delaying health maintenance far outweighs risks at present.

ABSTRACT

Background The coronavirus disease 2019 (COVID-19) pandemic disproportionately affects individuals with hypertension and health disparities. Methods and Results We assessed the experiences and beliefs of low-income and minority patients with hypertension during the COVID-19 pandemic. Participants (N=587) from the IMPACTS-BP (Implementation of Multifaceted Patient-Centered Treatment Strategies for Intensive Blood Pressure Control) study completed a telephone survey in May and June of 2020. Participants were 65.1% Black and 59.7% female, and 57.7% reported an income below the federal poverty level. Overall, 2.7% tested positive and 15.3% had lost a family member or friend to COVID-19. These experiences were significantly more common in Black (3.9% and 19.4%, respectively) than in non-Black participants (0.5% and 7.8%, respectively). In addition, 14.5% lost a job and 15.9% reported food shortages during the pandemic. Most participants complied with stay-at-home orders (98.3%), social distancing (97.8%), and always wearing a mask outside their home (74.6%). Participants also reported high access to needed health care (94.7%) and prescription medications (97.6%). Furthermore, 95.7% of respondents reported that they continued to take their regular dosage of antihypertensive medications. Among the 44.5% of participants receiving a healthcare appointment by telehealth, 96.6% got the help they needed, and 80.8% reported that the appointment quality was as good as or better than in-person visits. Finally, 88.9% were willing to return to their primary care clinic. Conclusions These data suggest that low-income patients, especially Black patients, were negatively impacted by COVID-19. However, most patients were able to access needed healthcare services and were willing to return to their primary care clinic for hypertension management. OBJECTIVES: Mississippi recorded the first case of coronavirus disease 2019 (COVID-19) on March 11, 2020. This report describes the initial COVID-19 experience of the single healthcare system serving Jackson County, Mississippi. The intent of this retrospective review of COVID-19 hospitalized patients was to identify any characteristics or interventions amenable to improving care management and clinical outcomes for patients within our community hospital. METHODS: All hospitalized patients 18 years of age and older in our health system with positive tests for COVID-19 (severe acute respiratory syndrome-coronavirus-2 [SARS CoV-2]) by reverse transcriptase-polymerase chain reaction between March 15 and April 10, 2020 are included in this retrospective observational report. RESULTS: During the study period, 158 patients of the 1384 tested (11.4%) were positive for COVID-19 infection. Of the 158 patients, 41 (26%) were hospitalized, with 17 (41%) admitted to the intensive care unit (ICU). The remaining 24 patients did not require ICU admission. The mean age of the 158 COVID-19-positive patients was 55 years (range 2-103). Obesity was noted in 68% of the hospitalized patients, including 13 (54%) of the non-ICU patients and 15 (88%) of the ICU patients. All 9 deceased patients were obese. Twelve of 17 patients received invasive mechanical ventilation (IMV) and 3 patients received only high-flow nasal cannula oxygen. Only 25% (3 of 12) of the IMV patients were successfully extubated during the study period. The median duration on IMV was 17 days (range 4-35). The mortality in the 158 COVID-19-positive patients was 5.7% (9 of 158). None of the 24 non-ICU patients died. The ICU mortality rate was 53% (9 of 17). CONCLUSIONS: This report describes a community hospital experience with COVID-19. Patient outcome was comparable to that reported at larger centers. Obesity was a major comorbidity and correlated with adverse outcomes. Amidst the initial wave of COVID-19 with high demand for inpatient treatment, it is reassuring that appropriate care can be provided in a community health system.

Background: Universities are at risk for COVID-19 and Fall semester begins in August 2020 for most campuses in the United States. The Southern States, including Mississippi, are experiencing a high incidence of COVID-19. Aims: The objective of this study is to model the impact of face masks and hybrid learning on the COVID-19 epidemic on Mississippi State University (MSU) campus. Methods: We used an age structured deterministic mathematical model of COVID-19 transmission within the MSU campus population, accounting for asymptomatic transmission. We modeled facemasks for the campus population at varying proportions of mask use and effectiveness, and Hyflex model of partial online learning with reduction of people on campus. Results: Facemasks can substantially reduce cases and deaths, even with modest effectiveness. Even 20% uptake of masks will halve the epidemic size. Facemasks combined with Hyflex reduces epidemic size even more. Conclusions: Universal use of face masks and reducing the number of people on campus may allow safer universities reopening.

ABSTRACT

As of December 14, 2020, children and adolescents aged <18 years have accounted for 10.2% of coronavirus disease 2019 (COVID-19) cases reported in the United States.* Mitigation strategies to prevent infection with SARS-CoV-2, the virus that causes COVID-19, among persons of all ages, are important for pandemic control. Characterization of risk factors for SARS-CoV-2 infection among children and adolescents can inform efforts by parents, school and program administrators, and public health officials to reduce SARS-CoV-2 transmission. To assess school, community, and close contact exposures associated with pediatric COVID-19, a case-control study was conducted to compare exposures reported by parents or guardians of children and adolescents aged <18 years with SARS-CoV-2 infection confirmed by reverse transcription-polymerase chain reaction (RT-PCR) testing (case-patients) with exposures reported among those who received negative SARS-CoV-2 RT-PCR test results (control participants). Among 397 children and adolescents investigated, in-person school or child care attendance ≤14 days before the SARS-CoV-2 test was reported for 62% of case-patients and 68% of control participants and was not associated with a positive SARS-CoV-2 test result (adjusted odds ratio [aOR] = 0.8, 95% confidence interval [CI] = 0.5-1.3). Among 236 children aged ≥2 years who attended child care or school during the 2 weeks before SARS-CoV-2 testing, parents of 64% of case-patients and 76% of control participants reported that their child and all staff members wore masks inside the facility (aOR = 0.4, 95% CI = 0.2-0.8). In the 2 weeks preceding SARS-CoV-2 testing, case-patients were more likely to have had close contact with a person with known COVID-19 (aOR = 3.2, 95% CI = 2.0-5.0), have attended gatherings† with persons outside their household, including social functions (aOR = 2.4, 95% CI = 1.1-5.5) or activities with other children (aOR = 3.3, 95% CI = 1.3-8.4), or have had visitors in the home (aOR = 1.9, 95% CI = 1.2-2.9) than were control participants. Close contacts with persons with COVID-19 and gatherings contribute to SARS-CoV-2 infections in children and adolescents. Consistent use of masks, social distancing, isolation of infected persons, and quarantine of those who are exposed to the virus continue to be important to prevent COVID-19 spread.
